Module overview
AZ Website Product Form - Product Inquiry / Contact Form on Product Pages
Add a configurable inquiry / contact form to every published product page in your Odoo Website/eCommerce.
Define fields (input, textarea, select, checkbox, hidden), placeholders, required/readonly state, and default values that can auto-fill from
product fields (e.g. [[default_code]] - [[name]]).
Label text accepts HTML links (e.g. privacy policy).
Submissions are stored as Inquiries with product, website, IP and user agent, and can trigger
email notifications to the website user and to an admin mailbox.
Supports multi-website, default form, and product-level include/exclude rules.
Field order is handled with drag and drop in the backend.
Perfect for lead capture, presales questions, and quoting workflows directly from product pages.
How it works
Install AZ Website Product Form from the Apps menu.
Configure forms under Website → Product Forms:
- Create one or more forms. Mark one as Default or assign forms to specific products. Use Include Products and Exclude Products to control visibility.
- Add fields and reorder them via drag and drop. Choose widget type (Input, Textarea, Select, Checkbox, Hidden) and input type (Text, Email, Tel, Number, Date, Checkbox, Hidden) when applicable.
- Set Label HTML (links allowed), placeholders, help text, and required/readonly flags.
- Use the Default Template to prefill values with product data (e.g. reference and name).
- Enable email notifications: auto-reply for the website user and admin alert with product reference/name included in the subject if desired.
- All submissions are saved in Website → Inquiries with product, website, IP address and user agent.
