Form 940 / 941 Payroll-Tax Worksheet — aggregate wages, withholding and employer taxes into IRS Form 941 (quarterly) and Form 940 (annual FUTA) worksheets with an EFTPS deposit schedule
Key Features
IRS Form 941 the employer files every quarter
IRS Form 941 the employer files every quarter and the IRS Form 940 it files once a year — and they have to tie out to the EFTPS deposits the employer already made. Doing that reconciliation by hand in a spreadsheet is slow, error-prone and

Form 941 quarterly worksheet — computes every Form 941 numbered line (wages/tips, federal income tax withheld, taxable Social Security wages and tips, taxable Medicare wages, Additional Medicare wages, total taxes before and after adjustmen
Employer-share tax engine
Employer-share tax engine — applies the combined 12.4% Social Security rate up to the annual wage base, the 2.9% Medicare rate and the 0.9% Additional Medicare rate on wages above the threshold, exactly the way the Form 941 instructions des
Form 940 annual FUTA worksheet
Form 940 annual FUTA worksheet — caps each employee at the $7,000 FUTA wage base, computes excess and taxable FUTA wages, applies the 0.6% net rate and handles state credit-reduction add-ons, with a transparent per-employee line.
Deposit schedule & EFTPS reconciliation
Deposit schedule & EFTPS reconciliation — classifies the employer as a monthly or semiweekly depositor from the lookback liability, computes the deposit due date for every pay date (including the $100,000 next-day rule) and reconciles filed
Monthly tax-liability breakdown
Monthly tax-liability breakdown — splits each quarter's total tax into its three monthly liabilities (Form 941 Part 2 / Schedule B summary) so the deposit obligation is obvious.
Wage entry register
Wage entry register — capture gross wages, federal income tax withheld, Social Security wages and tips, Medicare wages and Additional Medicare wages per employee per pay date, with the per-entry tax computed automatically.
Bulk wage entry
Bulk wage entry — key a whole pay period's payroll register into a single grid and create every wage entry for the chosen employer and pay date at once.
One-click worksheet build
One-click worksheet build — pick an employer, quarter and year, preview the matching wage entries and distinct employee count, and build (and optionally compute) the worksheet in a single step.
Filing deadline reminders
Filing deadline reminders — a scheduled action flags quarterly and annual returns that are not yet filed as their filing deadline approaches.
Filing lock & integrity hash
Filing lock & integrity hash — filing a worksheet freezes its wage entries and stamps a SHA-256 hash of the reconciled figures so a filed return can never be silently altered.
Role-based access
Role-based access — clerks maintain wage entries and deposits while managers own the worksheets and the filing, enforced by record rules.

Why Choose This Module
This app turns that reconciliation into a clean, repeatable, fully auditable workflow inside Odoo. Payroll clerks capture one wage entry per employee per pay date — gross wages, federal income tax withheld, Social Security and Medicare wages and tips. The worksheet then rolls everything up into the exact Form 941 numbered lines, applies the current Social Security wage base, the combined 12.4% Social Security rate, the 2.9% Medicare rate and the 0.9% Additional Medicare rate, and computes the monthly tax liability that drives the deposit schedule. The Form 940 worksheet caps each employee's wages at the $7,000 FUTA wage base, applies the 0.6% net FUTA rate (with state credit reduction where it applies) and reconciles against the deposits made. Nothing is hidden, nothing is faked, and every figure traces back to the wage entries that produced it.
