File Upload and Signature In Survey

This module enhances the Survey functionality by enabling users to upload files and capture signatures directly in survey forms. Multiple file attachments are supported, making it easier to collect detailed responses and approvals.

To perform a Single File Upload:
Navigate to Surveys > Surveys.
Create a new survey and add your questions as needed.

In your survey, create a question and set its question type to "Upload File".
This allows respondents to upload Single file as their answers to the survey question.

Create another question in your survey and set its question type to "Signature".
This allows respondents to provide their signature as an answer to the survey question.

Start the survey.
In the "Upload File" question, you will see an Upload button where you can upload documents.
You can also delete any uploaded documents by clicking the Delete button.

In the "Signature" type question, you have three options to provide an answer.
You can enter text in the Name field to generate an auto-signature.

In the "Draw" tab of the Signature question, you can draw your signature manually using your mouse or touch input.

In the "Upload" tab of the Signature question, you can upload a file containing your signature.
This allows respondents to provide a pre-made signature instead of drawing it manually.

Once the survey is completed, you can review all submitted answers in the "Review Answers" section.
This allows you to see responses including uploaded files and signatures.

For a Multiple File Upload, you need to enable the "Allow Multiple File Upload" option while creating the survey question.

This allows respondents to upload more than one file as answers to the question.

After uploading files in a Multiple File Upload question, you can review all uploaded files to ensure they are correct.
This helps respondents confirm their submissions before completing the survey.
